Safety Release Waiver

By registering for this workshop, you agree to the following: We are delighted you have chosen to enroll in our Program. We know this will be a meaningful learning opportunity! As a participant, or a parent or guardian of a participant, we want you to know and understand the common hazards of outdoor learning and textile arts processes before we begin our journey together. Through participation in this workshop you or your registrant could be exposed to bees, ticks, poison oak, sun, and the unexpected in nature. We will also be walking throughout gardens and across earthen terrain which could result in tripping and falling due to uneven topography. During the program, we may also cross roads in a carefully supervised group. Though occurrence is incredibly infrequent the outdoors is also home to many wild creatures and unexpected exposure to these animals could lead to injury or illness. In this workshop, students will be working hands-on with various tools inclusive of pruners, hot plates, high temperature substances, sewing needles, scissors, etc. In the case of an emergency that cannot be treated with simple first aid, it is Program Policy that Emergency Medical Assistance (911) be called which may require an ambulance. Lead Teachers are CPR and First Aid trained and equipped with first aid kits and may address any minor injuries such as cuts and scrapes. By agreeing to this waiver you confirm that you and/or your child are covered by valid health insurance, and acknowledge the inherent risks involved in participation in this program. By enrolling your child in our Program you assume such risks yourselves as well as on behalf of your child or registrant. Thus, by signing this form you agree that neither Melody Overstreet nor any of her Associates or Landlords are in any way legally or financially responsible for any injury, death, or illness, mentioned or unmentioned above, sustained by you or your child through participation in the Program and that you willingly consent to participation in the program. Thank you. By agreeing you also consent that the program may take photos of the workshop which might include your child. These photos are for professional and outreach use only and will never use a child's name. Lastly, by registering you agree to be supportive of your child exploring nature, working with natural dye plants, and sometimes getting dirty!
